It is strongly recommended that you read the Personal Insolvency Information booklet before you complete any of the forms below. The booklet provides important information on some of the consequences of presenting these forms to ITSA. There are important threshold amounts which you need to consider when contemplating bankruptcy, a debt agreement or a personal insolvency agreement. These amounts are indexed and the current amounts are available here.

You need to download each of the forms/documents shown in RED for each type of administration.
For example, if you want to become bankrupt, you need to download the
Information and Instructions, Debtor's Petition, and Statement of Affairs. These forms/documents need to be attached together at the time of lodgement.

Read all instructions carefully and complete all the questions on the forms. Incomplete forms will be returned to you. If you do not understand a particular question, email info@itsa.gov.au or call us on 1300 364 785 and we will be happy to assist.

Prescribed Information
ITSA strongly recommends that you get the assistance of a financial counsellor, debt agreement administrator, or registered trustee when proposing a formal debt agreement.

Proposing a personal insolvency agreement
Information & instructions
To propose a personal insolvency agreement, you are required to appoint a Controlling Trustee. You should contact a registered trustee to get their assistance in filling out the necessary forms.
